    Anaemia and its associated risk factors among pregnant women in the Gambia.

    Background: Anaemia is a global health problem affecting about a third of the world’s population. In pregnancy, it is a public health concern with consequences for mothers and infants, including maternal death and infant mortality. In low-income countries, 25% indirect maternal mortality and 30% neonatal deaths are due to anaemia in pregnancy. This study aimed to determine the prevalence of anaemia among pregnant women in The Gambia and identify common risk or predisposing factors associated with anaemia in pregnancy among them. Methods: This was a cross-sectional study of 180 consenting pregnant women, who attended two secondary-care level hospital for antenatal care between March and May 2023. They were selected by using a simple random sampling technique. Data were collected using a validated structured questionnaire and participants’ current antenatal cards while SPSS software version 26.0 was used for analyzing data. Results: The findings of this study shows that 21.7% and 8.3% of the pregnant women were anaemic at booking using 11.0g/dl and 10.0g/dl as cut off respectively with the mean haemoglobin concentration of 11.76+-1.36g/dl. The study revealed that anaemia during pregnancy is associated with factors such as educational level (p=0.048) and the average income per month (p=0.039). Majority of the women, (166, 92.2%), have taken malaria prophylaxis but only 95 (52.8%) admitted to sleeping under insecticide-treated mosquito nets which shows the low prevalence of Malaria among the participants (5.6%) Conclusion: The prevalence of anaemia among pregnant women in the Gambia is 21.7% which implies that anaemia is still a public health problem. Therefore, it is important for actions like early booking and improvement on health and nutritional education to be taken in order to reduce anaemia among pregnant women.

    Risk of anaemia in HIV positive pregnant women in Ibadan, Southwest Nigeria

    Anaemia in pregnancy is an important cause of maternal and neonatal mortality. It is a recognized co-morbidity of HIV infection. This study aimed to determine the risk of anaemia in HIV positive pregnant women. Methodology- This is a cross sectional study of healthy pregnant women attending Adeoyo Hospital, a secondary health centre in South-western Nigeria over a 1- month period (January 2007). During the study period, 2,737 eligible women presented for antenatal care. About 98% (2,682) of these women consented to HIV testing. Over all, their mean (± S.D) packed cell volume was 30.96% (±4.13). The prevalence of HIV infection was 2.9% (95% Cl 2.3% - 3.6%) and the overall prevalence of anaemia was 33.1%. Frequency of anaemia was significantly higher in HIV +ve women (57.3% vs. 42.7%, p= 0.00. OR=2.81., CI=1.72-4.58). HIV +ve women presented more frequently with moderate or severe anaemia. In the logistic regression analysis only HIV infection (OR=2.4, 95%CI=1.37- 4.21) and primigravidity (OR=1.25,95% 0=1.04-15.2) remained independently associated with anemia. Anaemia is common in HIV positive pregnant women in this environment. Care providers must endeavor to determine the HIV status of every pregnant woman especially if she presents with anaemia with a view to providing appropriate interventions.

    Blood transfusion requirement during caesarean delivery: risk factors

    Background: Group specific blood is often cross-matched ready for all patients scheduled for caesarean section in anticipation of haemorrhage during the surgery. This study was conducted to determine the risk factors for blood transfusion during anaesthesia for caesarean section. Methods: This was a prospective cross-sectional study. A total of 706 pregnant patients scheduled for emergency or elective Caesarean section at the University College Hospital, Ibadan, Nigeria between March and August 2011 were recruited. Participants were followed-up from the date of delivery till the end point of the study which could fall into either of the following conditions: satisfactory postoperative clinical status up to 48 hours post-delivery or death. Transfusion rate was determined and Chi-square test was used to determine if there exist an association between blood transfusion status and preoperative haematocrit level, years of experience of obstetrician, indication for Caesarean Section(CS), CS type (primary or repeat) and HIV status. Results: Transfusion rate was 9.1 %; variables found to be significantly associated with blood transfusion were; preoperative haematocrit less than 26%, increasing parity, years of experience of resident obstetrician, indication for CS (bleeding or not bleeding) and estimated blood loss. Being HIV positive does not increase the need for blood transfusion. Conclusion: Preoperative anaemia, increasing parity and severe blood loss at surgery significantly contribute to the requirement for blood transfusion in patients undergoing caesarean section.
